What is Combat Isolation?
Loneliness and isolation can have a huge impact on wellbeing — especially when living with a mental health condition. Our Combat Isolation Service offers short‑term, one‑to‑one support for adults aged 50 and over who are living with a mental health diagnosis and open to a community mental health team. The service provides you the opportunity to improve your confidence, increase your independence and reduce loneliness and social isolation.
We’re here to help people feel more connected, more confident, and less alone.
What the Combat Isolation Service Provides:
For up to 12 weeks, one of our Inclusion Support Workers will provide friendly, practical, and emotional support.
We can help with:
- Building confidence to get out and about
- Attending groups or community activities
- Encouraging independence and improving wellbeing
- Reconnecting with hobbies, interests, and social opportunities
- Companionship and regular conversation
Support is always person‑centred and guided by what matters most to the individual.
Who Is the Service For?
You can access the Combat Isolation Service if you:
- Adults aged 50 and over
- Open to community mental health services
- Diagnosed mental health condition
- Resident of Liverpool, Sefton or Kirkby
Referral‑Only Service
Please note: this service is referral‑only and must be made by a professional.
We are unable to accept self‑referrals from individuals or family members.
Eligible referrers include (but are not limited to):
- Mental health teams
- GPs
- Social workers
- Support coordinators
- Voluntary sector organisations
How the Service Works
Professional Referral
A referral must be completed by a health or social care professional. Self‑referrals are not accepted.
Initial Assessment
We’ll speak with the individual to understand their needs and what support would help.
Waiting List
Due to incredible demand, we currently have a waiting list. We will keep in regular contact with you throughout your time on the waiting list.
Allocated Inclusion Support Worker
An Inclusion Support Worker will be assigned to work with you.
Regular Visits
Your Inclusion Support Worker can meet with you on a weekly basis for up to 2 hours per visit. Visits are arranged with you at a convenient time and focus on support planning, confidence building and exploring your local community.
Planning Ahead
Toward the end of the service, we help individuals stay connected with community groups and ongoing support options. We can make onward referrals to other suitable support service where needed.
Volunteer
If you would like to engage with ongoing companionship, you can be matched with one of our dedicated volunteer befrienders. A volunteer can continue to visit you regularly, whether that be having a coffee at home or going out into the community. We match clients with volunteers based upon mutual interests and experience.
